Printable Recertification Application

If you wish to bypass using our online Self Service Portal and instead apply for Recertification using a printable application submitted via fax, email or mail, please follow the below instructions:

1. Download and print the application (RPSGT, CCSH)
2. Complete the application (to review our Privacy Policy, please click here), reaffirm your commitment to the BRPT Code of Conduct and select the Renewal Option that you are eligible for
3. Attach copies of certificates/transcripts of all CEC’s you wish to submit for your renewal
4. Attach a copy of your BLS/CPR
5. Attach a money order or cashier’s check, or provide your credit card information on the payment page for your renewal fee + any other additional fees that may apply to you
6. Submit it to the BRPT:
